Tim is used to real medical facilities, with lots of rooms and machines, ancient and modern. After all this seeing that all medical stuff this camp has fits into one little tent makes him sad. Inside the tent twins are playing a board game. Rikter brought it from their home to entartain his injured brother.

Doctors Without Borders -- I guess it's French because I've only heard the French acronym. They're a group that provides medical care in parts of the world too war-torn/poor/remote to have access to good hospitals.
I'm sure some of them have made similar expressions as Tim. :p
